Engine Room Fire on Bulk Carrier

What do you do when there’s a fire on-board? Read on to find out how the crew dealt with the incident and what lessons can be gleaned from it.

On 28 May 2021, a fire broke out near the incinerator of a loaded bulk carrier.

The fourth engineer started the incinerator on the second deck for burning sludge oil; 10 minutes later, there was a high temperature alarm on the incinerator panel. The fourth engineer adjusted the fuel supply to reset the alarm and went down to the bottom platform for his regular rounds.

The fire alarm sounded approximately 15 minutes later and all crew in the engine room saw flames and rapidly spreading thick black smoke near the incinerator.

The chief engineer pushed the manual activation button of the vessel’s hyper mist system for incinerator from the panel next to the engine control room. He noticed the “low control air pressure” alarm on the hyper mist system panel in the fire control room and the system deactivated. The chief engineer then tried to release the carbon dioxide system remotely from the fire control room and then locally but was unsuccessful.

Consequences

Several engine room equipment and power cables of mooring winches, boiler and steering gear were damaged.

There were delays for replacing the main engine’s sump oil and later for restoring power supply to mooring winches.

The cost of repairs by shore workshops, class attendance and engine room cleaning to remove soot came up to a total of USD$286,000.

Causes

The fire was caused by a combination of the following causes:

  • Incorrect, inadequate tools and equipment: On this incinerator, the exhaust fan generates negative pressure in the furnace and ensures that flames remain inside the furnace. A pressure switch monitors the negative pressure and trips the burner if the pressure increases to -10mm WC. This pressure switch apparently failed to trip the burner when the fan stopped due to suspected breaking of the V-belts. Failure of positive pressure inside the furnace, caused flames to shoot out of the incinerator flaps.
  • Lack of risk perception and risk awareness: The waste oil tank insulation which was about 1 meter away was oil-soaked and this combustible material caught fire instantly.
  • Inadequate inspection interval: Although the Planned Maintenance System (PMS) included a quarterly job for testing of safety functions of incinerators, the job description was not detailed/specific.
  • Inadequate emergency preparedness: The ship staff failed to follow correct procedures to release the carbon dioxide system. The pilot cylinder valve seal was found intact, and all cylinders were found full except the starter cylinder which was slightly empty. The pilot line for releasing the main carbon dioxide bottles was found to be disconnected. The safety pin was in place and not removed before attempting to release carbon dioxide locally. This pin was probably fitted during the last dry dock about 2 years back as a safety precaution to prevent unintentional release.

Crew must always confirm the PMS covers all required incinerator safety tests in detail and test all alarm, trips, interlocks. Seafarers must endeavour to check for oil-soaked laggings which may be painted over; if they find any such laggings, they must report it to the office and arrange to replace it at the earliest opportunity.

Ship personnel must keep all fire doors always shut and remove any arrangements to keep the doors lashed in open condition. They should conduct monthly checks to ensure the fixed fire extinguishing CO2 system’s discharge piping and pneumatic tubing are free of damage and properly connected.

In addition, conduct proper training and awareness of crew to release and operate the fixed fire extinguishing CO2 system from all locations regularly. During normal service, the safety pin to prevent unintentional release must not be in place.


Grounding of Chemical Tanker

Having a pilot on-board a vessel is necessary for certain maritime procedures. The ship’s officers and crew must remain alert and be ready to undertake necessary corrective action in the case of any emergency.

A chemical tanker was grounded during a piloted passage through a ship channel. The ship was in partly loaded condition and shifting with a pilot on-board.

The pilot asked for hard to starboard when port alteration was needed to bring the ship towards the middle of the channel. The bridge team immediately identified the mistake. However, the master did not override the pilot immediately. Instead, he was trying to voice his concern to the pilot and the helm correction was delayed by about a minute, resulting in the grounding.

The ship was re-floated with the help of two tugs after about 4.5 hours and brought to lay-by berth for inspection. Shortly after the grounding, the pilot showed paralytic symptoms.

The pilot’s wrong helm advice could be an indication of his illness; his paralytic symptoms were severe after the grounding, and he was hospitalized. Before the grounding, the pilot never mentioned if he was unwell.

Consequences

The vessel was delayed at port and off-hire for an unspecified amount of time. There was paint damage at the ship’s bottom. Cost was incurred for re-floating the ship, underwater inspections, surveys and services.

Causes

The grounding was caused by the following factors:

  • Extreme judgement and decision demands: The pilot asked for hard to starboard when the need was for a port alteration to bring the ship towards the middle of the channel. The bridge team immediately identified the mistake, but the master did not override the pilot’s instructions immediately. Instead, he was trying to voice his concern to the pilot that caused a delay in the helm correction by about a minute
  • Illness: The pilot’s wrong helm advice could be an indication of some illness though he never mentioned that he was unwell. His paralytic symptoms were severe after the grounding, requiring hospitalization.
